Survey Opportunity: Provider Provision of Condoms
Dear Members of the Vermont NP Association,
We would like to invite you to participate in a brief survey on providers' practices and perspectives on the provision of condoms and sexual/reproductive health care to adolescent patients in the medical home. If you currently provide primary care services to any adolescent patients in the age range of 13-21 years, and are at the level of physician (including residents/fellows), nurse practitioner, or physician assistant, you will qualify to participate in this study.
This study is being conducted by the University of Vermont Children's Hospital, and has been approved by the UVM IRB. To date, little is known about providers' practices or perceptions about potential risks/benefits of distributing condoms to adolescents in the medical home. We hope that by better understanding these factors, we can identify whether condom distribution in the medical home is an area of need, and whether providers would be interested in free or low-cost resources to help them begin offering this service in their practice.
Completion of this survey is voluntary. If you choose to participate, it should take about 5 minutes to complete. All responses will be de-identified and confidential. As a thank you for your participation, at the end of the survey, you will have the opportunity to enter your contact information for raffle of a $100 Amazon.com gift card. Your contact information is optional and you are welcome to complete the survey without sharing this information.
