VCHIP ICON survey for Pediatricians and Family practice providers

This month, the Improving Care for Opioid-Exposed Newborns (ICON) team at the Vermont Child Health Improvement Program (VCHIP) is conducting a short survey to assess community supports and barriers in caring for opioid-exposed newborns and their families in the outpatient setting. Included in this survey are also questions about the utilization of the Vermont Plan of Safe Care. Please take a few minutes to complete this short survey, your answers will be used to identify service delivery gaps and future process improvement work.
