Good Afternoon VHEPC Members & Partners,
The VHEPC Patient Transfer Coordination Support Workgroup meeting members identified the need for a statewide list of patient transportation resources for the state of Vermont, as one did not currently exist. Then the VHEPC Executive Committee voted for All Clear/VHEPC staff to create the Vermont Medical Transportation List. Due to current weather events, we wanted to share this information with you immediately, however, we will be walking members through how to use this tool during the Q1 Full Coalition Meeting on August 8th (email us for more information about the meeting).
The dashboard contains the following details; (1) service area, (2) payor limitations, (3) days and hours of service, (4) limitations of type of patient to transport, (5) number of units, (6) capacity of units, (7) contact Information, (8) MOUs, and more!
The Vermont Medical Transportation Dashboard can be accessed, HERE. (https://app.smartsheet.com/b/publish?EQBCT=d75f7304bf62472c809beb43a7cbd29d)
Anyone can access the dashboard on our website with the link, HERE. (https://www.vhepcoalition.org/vermont-medical-transportation-resource-list)
***DISCLAIMER: This tool does NOT encompass ALL medical transportation services across the state. This tool will continue to evolve, we will be taking ongoing feedback and making changes to fulfill member and partner needs as time progresses. The agencies and organizations included in this list were found during preliminary research. If your agency/organization provides non-emergent or interfacility transfers and are not included in this list, please fill out the survey below:
VHEPC staff will update current and new entries to the list as you all complete the survey based on your own agency/organization.***
We encourage you to share this resource with anyone who could benefit from it. For those of you who have not signed a free membership agreement, you can do so, HERE. Being a member gives you and your organization immediate access to the tool, integrates you into the planning process and provides updates as needed.
Please reach out if you have any questions or concerns. We are always here to help!
Thanks,
VHEPC Team
