5. Anyone who wants to refresh their knowledge or has come across challenging conversations with patients and families around advance care planning
6. Anyone interested in tips on starting a conversation with patients and families about POLST

What will be covered?

1. Phil Lawson MD and Michele Lovell, DNP will provide a brief overview of Advance Directives versus Portable DNR and POLST Medical Orders.  They can help you with conversation strategies to identify patient goals of treatment. Most of the hour will be devoted to answering attendee questions and sharing challenging situations.
