Research on Overweight and Obese Children

Dear Participant,

My name is Sarah Plunkett and I am a nurse practitioner student at the University of Vermont. I am conducting a research study regarding healthcare providers' attitudes, beliefs, and patterns of diagnosis and management of overweight and obese children. The link to the survey can be found below. The survey contains 20 multiple choice and short answer questions. The survey will take approximately 20 minutes to complete. This survey is voluntary and no personal information will be asked. There will be no link between your email address and the survey.

There are no anticipated risks to your participation or direct benefit to you. The information you provide might aid in future research. The data collected from the survey will be used for my thesis. By initiating and submitting the survey you are providing voluntary consent for the use of your data in this research.
