Long-Acting Reversible Contraception Survey: Final Reminder

Dear Health Care Provider,

Recently you received an invitation to participate in a research survey on health care providers' knowledge and practice around Long-Acting Reversible Contraception (LARC). Thank you very much if you have already filled this out. If not, please consider participating by clicking on the link below. This survey will help us determine educational needs of provider on LARC, and assist us in creating trainings on this topic.

The survey should take about 15 minutes to complete, and will be open for one more week until July 7th. We thank you in advance, and appreciate your participation in this important work.
